Brow Descent or Excess Eyelid Skin?
This distinction is the most critical step in planning around the eyes, and it is often skipped.
A heavy, full upper eyelid can have two different causes: excess skin on the lid itself, or descent of the brow. When the brow drops, all the tissue above it comes down too and reads as excess lid skin.
A simple check: in the mirror, lift your brows a few millimetres with your fingers. If the eye area opens noticeably and the heaviness on the lid disappears, the real problem is brow position.
Why does this matter? Because removing skin from the upper lid while the brow is low can pull the brow down further and worsen rather than improve the appearance. The correct order is to position the brow first, then assess the lid if still needed.

Forehead Lines and Brow Position
In people with brow descent the body develops an unconscious compensation: the forehead muscles work constantly to hold the brows up.
This has two consequences:
- Horizontal forehead lines deepen. These lines are the mark not only of ageing but of continuous muscle use.
- A feeling of fatigue develops in the forehead and around the eyes by the end of the day. People usually describe this as “my eyes feel heavy”.
Once brow position is corrected, the need for that compensation disappears and the forehead muscles no longer have to work continuously. This is the aspect in which the procedure contributes to daily comfort, not only to appearance.
Note: existing forehead lines are not erased by the procedure. Their deepening slows because muscle use falls, but established lines require separate skin-directed approaches.
What Is a Natural Brow Position?
The most common concern with this procedure is a “startled” look. That is the result of over-elevation, not of the technique.
A natural brow position has measurable criteria:
- The highest point of the brow should sit not at the inner corner but toward the outer third.
- The inner end of the brow generally stays where it is; raising it there produces the startled expression.
- Natural brow position and arch differ between women and men — in men a flatter, lower brow is expected.
- The change should be millimetric; the aim is not to alter the facial expression but to produce a rested appearance.
